HOSPICE OF NEW YORK
—Provide psychotherapy/grief counseling support to patient and patient families.
—Provide music psychotherapy support and comfort care
—Utilize appropriate referral protocol for population
—Observe clients in music and non-musical settings to set appropriate therapeutic goals
—Provide in-service to educate staff on Creative Arts Therapy/Music Therapy (as well as the psychotherapeutic components)
—Families in the treatment planning and treatment process when appropriate
—Provide written documentation that demonstrates evidence based outcomes related to goals/interventions.
—Primary caseload diagnosis: Parkinson’s, Dementia, Alzheimer’s, Cancer, Multiple Sclerosis, and Congenital Heart Disease (patients with a six month prognosis)
—Work with a five person interdisciplinary treatment team that includes Nurse, Physician, Social Worker, Music Therapist and Chaplain to provide Holistic Care

–Treatment goals implemented to decrease impulse behavior/improve reality orientation
–Assist patient in developing patients perceptual and cognitive skills

–Provide pain management treatment subsequently reduce patient agitation
–Partake in bi-weekly interdisciplinary group meetings with treatment team members
–Weekly text messages/phone calls to patient family members on status update
–Provide grief counseling
–Procedural support/music therapy at funerals
